CONSENT AND RELEASE FORM
I, the undersigned entry / member, hereby acknowledge that I am aware of the strenuous physical exercise involved in participating in the APLUS MARTIAL ARTS PROGRAMS & SERVICES. I hereby consent to hold the sponsors free of any or all liability, claims, or actions whatsoever, arising from any injuries, accidents, illnesses, death etc., due to the attendance at APLUS Martial Arts TKDD Programs and Services. I hereby consent to allow the sponsors to take such action as is necessary to contact and provide emergency and medical assistance. NO CHILD WILL BE ALLOWED TO PARTICIPATE IN ANY PHYSICAL ACTIVITY WITHOUT PARENT CONSENT. CHILDREN MUST HAVE A SIGNED CONTRACT IN ORDER TO PARTICIPATE IN ACTIVITIES. EVERYONE MUST REMOVE SHOES BEFORE ENTERING THE MAT AREA.
Recognize and understand that martial arts/ fitness/ sports training is an activity that involves physical contact and that my participation might result in serious injury, including permanent disability or even death, and severe social and economic loss.
2. Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises, or of any of the equipment used.
3. Recognize that there may be other risks that are not known to me or to others or not reasonably foreseeable at this time.
4. Agree to inspect the facilities, equipment and pairings prior to participation. I will immediately inform an instructor if I believe that anything is unsafe or beyond my capability and refuse to participate.
5. Assume all the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or death.
6. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training and competition.
7. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/or competition.
8. Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with medical assistance or treatment for such injury.
9. Release, waive, discharge and covenant not to sue, APLUS Martial Arts TKDD LLC., its affiliated organizations and governing bodies, their officers, instructors and personnel, other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise.
I HAVE READ THE ABOVE WARNING, WAIVER, RELEASE, AND ASSUMPTION OF RISK. I FULLY UNDERSTAND ITS CONTENTS, AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I HEARBY SIGN IT VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY.
Please inform staff of any health conditions or injuries.
Parents or guardians give consent upon enrolling their child/family.
Handbook
All tuition is due Friday in advance for the next two weeks. As of Friday, at 6:30 p.m., your tuition is considered late. A late fee of $25.00 is added to your account on Friday of every week. If tuition is more than one week late, your child may terminated from our program. APLUS will not hold a place for your child in its program if payment is not immediately made. Tuition is due regardless of attendance, in addition to school closings, holidays, and winter and spring break.
A four-week written notice must be given to withdraw any child from APLUS Programs.
APLUS Martial Arts & Athletics reserves the right to take civil action against any open accounts that are past due. To book a spot for the following school year, Buyer must register for the next school year before the current school year ends; otherwise, admission is on a first-come, first-serve basis.
Other Programs and Activities
APLUS offers camps when schools are closed all day. We require ten students to be signed up. $15 for after school, $30 for night class & $40. for drop-in.
After School Program objective is to provide an active environment where students can learn and develop through martial arts, physical fitness, and sports. We will try to monitor and check homework, but students may not have time to complete the task due to the class schedule. We ask parents to review homework assignments. If a student does not have time to complete homework, we cannot be held responsible for the incomplete assignments.
Arrival/Departure:
Upon arrival and departure, parents are to sign their child in/out in their car as they wait for the child to enter the building and wait for the athlete to have a health check and be given the okay by staff before leaving. All athletes must be checked in and out by an authorized adult. An emergency phone number should be updated on the family's account. You must list all individuals picking up your child on the emergency form. If someone other than the persons listed under "authorized to pick up" comes to pick up a child, the child will not be released to that person unless we receive a phone call from the parent or legal guardian. If it is the first time that that individual has picked the child up, an ID will need to be shown at the time of pick up, even if the individual is listed under emergency contact.
Your child's safety is our priority. Parents can call for their children to meet outside. Please call if someone other than a parent is picking up your child. They must come to the door and show I.D. If unsure, we will contact the parent; for the child to be released, we will not remove a child if the person is not on the pickup list.
Illness of a Student:
APLUS is not authorized to care for sick children. Parents are responsible for providing an alternate plan for a child's illness. Children with allergies or other conditions will not be excluded from any activity. We are an active program. You can provide an alternate plan if your child participates in an activity. APLUS will not administer any medication to any athlete. This does not include prescription inhalers. If an athlete is ill when he/she arrives at the studio, the athlete will not be allowed to enter.
If an athlete becomes ill while at APLUS or activity destination, the parent/guardian will be notified immediately, and the athlete will need to be picked up from that destination. Any athlete will not be re-admitted to the studio after illness unless the parent/guardian presents a written statement from the physician certifying that the athlete is well enough to return to APLUS.
If your child has health conditions, please inform our Director.
STUDENT CODE OF CONDUCT
The following represents an outline of acceptable behavior during part of our program. This code of conduct is subject to change and may be modified at the sole and exclusive discretion of APLUS Martial Arts TKDD. Reasonable notice shall be granted whenever possible; however, in some instances, immediate policies may need to be enacted whereby no notice is practical. As a policy, we intend to write pertinent information regarding any change in these rules and regulations to all parents.
Rules and Conduct
1. No FIGHTING, horseplay, aggressive contact with anyone, or other rowdy, disruptive behaviors permitted.
2. No foul language or other inferior language will be tolerated.
3. No toy weapons of any kind would be allowed.
4. Students are to follow the instructions of the program director or martial arts instructor while under our guidance.
5. Students are to use the cubby areas to store their belongings while in the program.
6. APLUS Tae Kwon Do assumes no responsibility for lost or misplaced items or items left behind after five days of students leaving this program.
7. Acts of dishonesty, deception, deceit, or other improper behavior shall result in the student's termination from the program. This is a ZERO tolerance policy.
8. Students may not bring buddies to "hang out" while in our program. Furthermore, students may not
have friends call upon them while programming. Your friend(s) will be instructed to go home.
9. Students may leave the program to walk home with the parent's express written or verbal consent. Parents must provide written notice if someone other than the authorized parents is picking up the student from the program.
10. There will be ZERO tolerance regarding any drug or possession of any narcotic into our studio—automatic dismissal.
11. Students should not enter the studio premises during the After-School program. (In case of an emergency or appropriate circumstance, the student will be accompanied by a counselor or staff member of A Plus Tae Kwon Do).
12. Students can arrive 5-7 minutes before class begins; students are to remain quiet until martial arts class begins. There will be ample time for other recreational activities during the program. The Program Director or Lead Instructor will plan these activities.
13. We do not allow relationships at APLUS. Students should be focused on other things.
14. Students will not disrespect staff or fellow students at any time, should it be verbal or physical.
15. Children must learn and follow TAE KWON DO rules (MAT RULES).
TRANSPORTATION INFORMATION
Students will be transported from their respective schools at a specific time. Therefore, each student must be ready to board the transport van / or bus at the time window. Drivers can't be held accountable for students not at their pickup point at their designated time. There will be a $15—return pickup fee if the van can be returned. If not, parents are responsible for pickup.
Students will be required, by law, to wear their seat belts always while being transported. They must use indoor voices and no horseplay. Middle school students may use their phones if it is not distracting. Phones are not permitted for ES students. Any student who violates this policy will be excused from our program. Students cannot bring bikes or other recreational hardware onto the transportation van as there is no room for these articles. These rules are designed with the safety of our entire student body in mind.
Please notify us if your child is too sick from school or goes home early from school. This is critical as waiting for absent children will disrupt our bus schedule. Please call or text APLUS as early as possible (Before 12:30 pm) at (240)2748489. Please feel free to leave a message.
*Please pack healthy light snacks for your child or give them money to buy a snack. This can be included in your tuition or paid daily. All students need a snack to keep their energy level up. Please make sure your child receives 8-10 hours of sleep each night.
*Please ensure all belongings are labeled so any lost items can be returned to them.
Students must have a clean uniform every day for class.
*We do not permit gum, candy, soda, or sunflower seeds at APLUS.
*Please call if you are late picking up your child. A late fee of $10.00 will occur at 6:01 pm and $2.00 every minute after that. Late payments are due in cash at the time of pick up.
HOLIDAY SCHEDULE
APLUS PROGRAMS WILL BE CLOSED ON ALL FEDERAL HOLIDAYS, Native American Heritage Day, Thanksgiving break (Wednesday-Sunday), Winter Break December 24-January 2nd, M.L.K birthday, Presidents Day, Spring Break, Memorial Day, Juneteenth and Fourth of July.
INCLEMENT WEATHER SCHEDULE
APLUS will be closed when the government is closed.
We will also follow their liberal leave plan when it comes to delays.
If schools close early, we will pick students up at that time.
Please help us by calling if your child is not in school.
If schools open 2 hours late and we are open for before care, we will only take drop-in students if we have space. Parents must call in advance.
The drop-in fee is $10 per student per day/$25 for the week.
If schools do not open, and we can, an all-day camp will be offered for $15 per child.
We will do our best to keep parents updated.
Dress Code
Students must have a clean uniform for each TKD and not participate in any activities if they do not have both uniform & belt. APLUS tee and fitness attire must be worn for Fitness Training. Athletes may not wear accessories during any activity on the mat. This includes but is not limited to dangling earrings, chains, bracelets, beads, and barrettes that must be secured in hair. Any loose hair accessories must be removed because they can cause injuries during training or play time, and long hair must be pulled back out of the face.
GENERAL HYGIENE
Here at APLUS, our students are very active and will sweat during classes. Please be sure that they are adequately prepared for physical activity. Having a fresh start each day and not forgetting to use essential hygiene products will allow your child to stay fresh all day. They keep backup products in their lockers that should not be shared. Please label all personal items.
Liability Release Authorization
Parents grant permission for your child(ren) to be transported by van to and from field trips. I permit my child(ren) to participate in all APLUS Martial Arts & Athletics activities/field trips. In case of an emergency, I understand that every effort will be made to contact me or my emergency contact. If you or someone on the emergency form cannot be reached, you permit APLUS to secure the medical treatment necessary for your child. You understand that APLUS assumes no responsibility for injuries or illnesses that your child may sustain due to his/her physical condition or resulting from his/her participation in any activity. You expressly acknowledge on behalf of yourself and your heirs that you assume the risk for all injuries and illness that may result from his/her participation in these activities. You with this release discharge APLUS MA&A TKDD, affiliates, and employees from any claims for injury, illness, death, loss, or damage that he/she may suffer as a result of his/her participation in these activities. APLUS is not responsible for any personal property lost or stolen. APLUS photographs and video of all participants for advertising purposes only. You acknowledge the waiver, accept the conditions above, and sympathize with the goals and purposes. You agree to adhere to and abide by the policies outlined in this agreement. You understand and agree to carry out the parent's responsibilities under this Policy and Procedures Agreement.
By signing, you have entirely read APLUS MA &A TKDD Guidelines and Procedures and acknowledge and understand all terms and conditions therein. You agree to complete financial responsibility for all weeks you have selected. You understand and accept that you are responsible for payments regardless of attendance. You agree to uphold all the terms and conditions of this document as they pertain to your child's enrollment in APLUS MA&A TKDD.
